Buccal Fat Pad Removal for a Contoured Face

October 25, 2018/0 Comments/in Buccal Fat Pad, Face Procedures, Facelift /Posted by Dr. Delgado
Buccal Fat Pad Removal

Before and After Buccal Fat Pad Removal

The buccal fat pad (BFP) is a encapsulated mass of fat located in the mid-face area just below the cheekbone. It is responsible for fullness in the cheeks and most noticeable in babies where it aids in sucking functions. The size of the BFP reduces as the child grows.

Buccal Fat Pad Causes Facial Fullness

Buccal Fat Pads in babies

The BFP can be the cause of facial fullness in adults; removal may be considered for any age group and done in conjunction with other facial procedures. Weight fluctuations may affect the size of the buccal fat pad which can vary from the size of a grape to the size of a golf ball.

For some people, the buccal fat pad can descend, adding to the jowl. In order to achieve a nice jawline with a facelift, it is essential for the plastic surgeon to identify the herniated fat pad for removal. If it is not removed, the jowl will only be partially corrected.

Removal of the buccal fat pad is usually done from the inside of the mouth. It can be done from the outside during a SMAS facelift, but Dr. Miguel Delgado, M.D. prefers the inside of the mouth as it is riskier on the outside due to the facial nerves in the area.

Buccal Fat Pad Commonly Missed

With less experienced plastic surgeons, the buccal fat pad is commonly missed leaving the patient with unsatisfactory results. It takes an expert cosmetic plastic surgeon to be on the lookout for the possibility of the descended buccal fat pad.

Do Your Ears Hang Low? Consider Earlobe Reduction

October 16, 2018/0 Comments/in aging earlobes, earlobe, earlobe reduction, Face Procedures, Non-Invasive, non-surgical /Posted by Dr. Delgado
earlobe reduction in San Francisco

                       Before and After Earlobe Reduction

Sagging and drooping skin are some of the most distressing signs of aging. Most people don’t think about their earlobes until the lobes become wrinkled and may become elongated.

Earlobes Droop as We Age

Skin contains collagen and elastin. The collagen and elastin begin to dissipate as we age, making the skin begin to sag and wrinkle. Without proper elasticity, the skin will look older, and the earlobes may elongate. For women who like to wear heavy earrings, the problem is amplified. It is difficult to hide the ears with hair.

Fat or Juvederm For a Non-surgical Solution

Earlobe rejuvenation is possible with Juvederm an injectable filler. Juvederm will add volume and plump the earlobes eliminating the sagginess and elongation. The filler can also reduce the earring hole if it is stretched. Juvederm injections take about 30 minutes or less with no downtime. As spectacular as Juvederm is, there is a more permanent solution, fat injections! Fat injections are becoming the most popular filler for the earlobes and the longest lasting.

Earlobe Reduction With a Facelift

For a more permanent solution, that is increasing in popularity, is a surgical reduction of the earlobes. Surgical reduction is especially popular for the facelift patient. Elongated aging earlobes can sabotage the results of an otherwise fabulous facelift.

Earlobe reduction can be challenging for the surgeon to achieve a natural look. Dr. Miguel Delgado, M.D. says the surgeon must have a good sense of balance, and an artistic eye. The incisions must be placed expertly so that when they are healed, they are well hidden. If the lobes have a wrinkled appearance, volume can be added with fat injections or Juvederm.

Aging Body Parts That Reveal Your Age First

Facelift surgery in San Francisco

                             Before and After Facelift Surgery

Aging gracefully is acceptable for some people, but looking older than you are is a problem for many, but help is available!

Aging Body Parts

With aging our skin thins and becomes less elastic resulting in wrinkles, lines, and age spots. Even if you have been diligent with moisturizers, using sunscreen, and avoiding cigarette smoking, some body parts are going to show aging before others.

The body parts that betray your age first are:
• Face
• Eyelids
• Neck
• Hands

FACE:

Aging face before and after Facelift San Francisco

                              Before and After Facelift Surgery

While freckles were cute for children, dark spots that appear as we age are not attractive. After a lifetime of sun exposure, age spots show up on the face, neck, chest, and hands. Age spots can be treated with prescription-strength retinol. For a faster treatment, Dr. Miguel Delgado, M.D. offers chemical peels, microdermabrasion, and laser treatments. Dr. Delgado has the Palomar Icon, one of the most popular laser treatments for removing age spots.

For reducing wrinkles, the longest lasting is a full facelift. For patients not ready for surgery, injectable fillers offer a non-surgical, minimally invasive solution for filling creases, wrinkles, and lines. Dr. Delgado has the most popular fillers available for his patients including Sculptra, Juvederm, Radiesse, Belotero Balance, Bellafill, and of course Botox.

EYELIDS:

Before and After Blepharoplasty

                   Before and After Eyelid Surgery

The muscles supporting the eyelids stretch and grow weaker as we get older. Fat may accumulate above and below the eyes. Patients need to stay hydrated, consume less salt, and get plenty of rest. However, eyelid surgery remains the most effective solution for aging eyes.

NECK:

Before and After Neck Lift Surgery

The neck often looks older than the face because the skin is thinner. You can hide the neck with scarves or turtleneck shirts, or opt for neck lift surgery that will turn the clock back many years.

HANDS:

Radiesse for Hands in San Francisco

                      Before and After Radiesse

Always exposed to the elements, your hands may be the first to show aging. We start to lose volume on the backs of the hands as we get older, with the loss of volume, the tendons and veins become more prominent. Radiesse has proven to be a great filler to plump up the hands and treatment with the Palomar Icon to remove age spots will return your hands to a youthful look.

General Anesthesia, is it Safe?

August 10, 2018/0 Comments/in blood pressure, General Anesthesia, heart rate, nausea, vital signs /Posted by Dr. Delgado
Dr. Miguel Delgado San Francisco

Dr. Miguel Delgado
Board Certified Plastic Surgeon

General anesthesia causes a lot of concern for many patients, especially those who have never had any type of surgery. Psychiatrists have done studies that show about 30% of people are more fearful of anesthesia than the surgery itself.

Understanding General Anesthesia

It is normal to fear the unknown, understanding a few facts about general anesthesia should help put your mind at ease.

Complications from general anesthesia are usually attributed to patients who are in poor health, obese, smoke, or have cardiovascular or other medical risk factors. These patients would not be approved for electives procedures. Even these patients who may need emergency surgery have only 0.01-0.016% chance of anesthesia causing a fatal complication.

Everyone reacts differently to anesthesia, when waking up it is not unusual to feel “loopy,” over-emotional, or having slurred speech. Most agree that time seems to go by in a blink, and many believe they haven’t had their surgery yet. General anesthesia wears off fairly quickly, but patients are advised not to make any major life decisions for 24 hours after surgery.

Vital Signs are Constantly Monitored

Due to some media stories and movies, some patients are very fearful that they may be awake and paralyzed while under general anesthesia. However, by monitoring all vital signs by the anesthesiologist, including blood pressure and heart rate, the doctor would be aware of any increases before the patient would regain awareness.

Dr. Miguel Delgado, M.D. is joined in the operating room with Dr. Craig Riddle, a Board Certified Anesthesiologist, at Marin Cosmetic Surgery Center.  Dr. Riddle calls all surgery patients before their surgery to discuss their medical history and answer any questions the patients may have.

The day of surgery, Dr. Riddle will start an IV drip and administer medications. You will soon feel very relaxed, and like you are in a deep sleep. You will feel no pain, nor will you remember anything after the medication takes effect until you wake up in the recovery room. Dr. Riddle will be monitoring your oxygen intake; circulation, vital signs, fluid loss, and level of consciousness the entire time of surgery to ensure all levels are normal and stable.

No More Nausea!

Some patients who have had general anesthesia before are concerned about feeling very nauseated upon waking. However, due to Dr. Riddle’s expertise with anesthesia, he rarely has patients experience this uncomfortable side effect. Many patients comment on how relieved they are that with Dr. Riddle there was no nausea!

Call Today!

General Anesthesia is safer today due to advances in both medication and technology. If fear of general anesthesia is holding you back from seeking your dream cosmetic procedure, you are invited to speak with Dr. Delgado and Dr. Riddle to address your concerns, give us a call at (415) 898-4161.

Facelift Scar Placement

        Before and After Facelift Surgery

Well placed facelift scars should not be evident to anyone except maybe your hairdresser. Deciding to turn back the clock and rejuvenating your face and neck is a personal decision. The hallmark of an excellent cosmetic plastic surgeon is one who can give natural results without any apparent signs that anything has been done. Depending on the type of facelift will determine the placement of the incision.

The Modern Facelift

The modern facelift is the most extensive but will last the longest. Generally, Dr. Miguel Delgado, M.D. places the incisions in the groove behind the ears and into the hairline. When done correctly, the scar is almost imperceptible once it has healed. However, for men, an entirely different technique is employed. Dr. Delgado takes into account the placement of tissue and facial muscles; he develops a customized plan for each patient. Of great importance is the male hairline. Incisions usually are placed in front of the ears, so that hair from the sideburns is not pulled into the ears. Recovery is a bit longer but well worth the final natural results.

The Short Scar Facelift

With this surgery, the incisions are in front of the ears. The short scar lift incorporates lifting the deep tissues, repositioning them, and removing some excess skin. This procedure is ideal for younger patients that do not have a lot of sagging or for a touch-up procedure for previous facelift patients.

The Mini-Facelift

The mini-facelift is not a long-lasting lift; it involves tightening of the skin only. Depending on the elasticity and texture of the skin determines if a patient will benefit from this procedure. The mini-facelift may be a suitable procedure for patients in their late 30’s to early 50’s, but patients need to be evaluated individually. Incisions are generally hidden around the ear and often within the tragus.

Common Myths About Breast Reduction Surgery

July 9, 2018/0 Comments/in Breast Procedures, Breast Reduction, Breast Reduction Benefits, Breast Reduction Patients, Breast Reduction Results, Breast Reduction Risks, Breast Reduction Surgery /Posted by Dr. Delgado
Breast reduction in San Francisco

Before and After Breast Reduction Surgery

Breast reduction surgery rates as one of the highest in patient satisfaction for elective plastic surgery. Surprisingly, there are several myths about the procedure that discourages many women from proceeding with surgery. Dr. Miguel Delgado, M.D. states that is unfortunate because most women who have undergone the procedure wish they had done it sooner as it significantly enhanced their quality of life.

Some of the inaccuracies that you should be aware of are:

Losing weight will reduce your breast size

•  Breasts are composed of both breast tissue and fat. Losing weight may affect the breasts a bit but won’t reduce breast tissue. Also, when you lose weight, you can’t know where you will lose fat. The ratio of fat to breast tissue is different for all women. For those women that do have a high percentage of fat, losing weight may help if you could count on the fat loss coming off your breasts. Otherwise, liposuction is a good alternative for these women.

You can’t breastfeed after breast reduction surgery

•  If you select an experienced Board Certified Plastic Surgeon, the chances are good that you will still be able to nurse if the nipple and areola are still connected to breast tissue. However, in some breast reduction cases, it is necessary to reposition the nipple/areola complex, and the ability to breastfeed may be somewhat compromised. With the passage of time, sensation may start to return, and lactation may be possible.

Women can choose their breast size.

•You can have a general goal for the desired breast size. However, some factors influence the final result, such as variations in surgery, and how the breasts heal. Showing your surgeon pictures of what size you are aiming for can be a big help, but results cannot be guaranteed.

Women should be happy with large breasts

•  Some men and women with small breasts don’t understand why a large breasted woman would want a reduction. Having breast reduction surgery is a very personal decision. Many women suffer from back and neck pain, shoulder grooving from bra straps, rashes, not to mention difficulty exercising, and shopping for clothes that will fit them. For many women, surgery is medically advised.

Dr. Miguel Delgado Named a Top Doctor by Sonoma Magazine

Dr. Miguel Delgado Top Doctor

Selecting a cosmetic surgeon can be a challenging endeavor. Beyond finding a list of Board Certified Plastic Surgeons in your area, and checking online reviews, patients are advised to view before and after pictures of the surgical procedure(s) that they desire. However, there is more that can be done to narrow the choices.

Dr. Miguel Delgado Top Doctor for the Third Year!

When viewing a potential surgeon’s website, note if there are various awards posted. Many cities have local magazines that publish many helpful topics for their reader’s. Sonoma Magazine has a lot to offer their reader’s. In addition to “things to do” there are recommendations for; restaurants, hotels, shopping, trip ideas, Top 100 Wines, and Top Doctors. 2018 is the third year the magazine has had the Top Doctors feature. Dr. Miguel Delgado, M.D. has been recognized each year including 2018, which will be out in September.

Sonoma Magazine uses a third party “Professional Research Services” (PRS) who compile their Top Doctors list. PRS believes the best referrals come from other doctors. They send out a survey to all licensed physicians in Sonoma, Napa, and Marin. The doctors are asked to nominate 3 physicians. They are to take into account; board certifications, education, bedside manner, and hospital appointments. The doctors cannot nominate themselves, or pay to be a Top Doctor.

All Top Doctors are Board Certified

PRS then reviews all of the nominations, verifying all of the submitted information. PRS checks that the doctor is Board Certified and licensed by the Medical Board of California. Additionally, the disciplinary records must be clean.

Besides being a tremendous guide for the city’s residents, newcomers will find the information invaluable as they get acquainted with their new home.

Treating Lip Lines

June 8, 2018/0 Comments/in Dermal fillers, Face Procedures, Facelift, Lip Augmentation, lip enhancement, non-surgical, skin care /Posted by Dr. Delgado

Lip lines can be treated

Peri-oral wrinkles are a term to describe the fine lip lines and wrinkles found around the mouth and the upper lip. We lose elasticity and collagen in the lips as we age. For some, it appears that the lips have disappeared entirely making people look sad, tired, or angry.

The lip lines should not be confused with smile lines or laugh lines that extend from the corner of the mouth to the corner of the nose. These lines are referred to as the nasolabial fold.

The main factors that contribute to lip lines are:

• Aging
• Smoking cigarettes
• Poor eating habits
• Excessive alcohol consumption
• Not enough sleep
• Too much sun exposure
• Lack of exercise

The good news is that lip lines can be reduced dramatically!

The Best Techniques for Fuller Lips

Fuller lips help minimize lip lines. The most popular procedures for the lips are:

• Synthetic fillers such as Juvederm which will last 9 to 12 months
• Fat injections with your own fat
• Augmentation with your own tissue is done many times in conjunction with a facelift and is Dr. Miguel Delgado’s preferred method. A piece of the SMAS tissue is shaped and inserted through the lips, and it can last for many years.

After You Have Turned Back the Clock on Your Lips

Protect your lips by:

• Using sunscreen with SPF 30 or more
• Applying a good moisturizer once or twice a day
• Exfoliate your skin once or twice a week
• Consume a healthy diet with plenty of vegetables and fruit that are high in anti-oxidants
• Take shorter showers. Showering can wash away natural oils that keep the skin hydrated
• Use a good quality cleanser, not soap that will irritate and dry out your skin
• Sleep on your back, side sleeping or on your tummy will contribute to lines on the face, including the lips

The Wonders of Radiesse

May 27, 2018/0 Comments/in Dermal fillers, non-surgical, Skin Care /Posted by Dr. Delgado

FDA approved Radiesse is known as an excellent dermal filler for facial wrinkles and creases. More recently Radiesse has been used for nose reshaping for patients who are not ready for surgery or want to know how they will look before committing to surgery.

Radiesse for Hands in San Francisco

Before and After Radiesse Treatment for the Hands

Another popular application for Radiesse is for the treatment of aging hands. As we age, it is normal to lose volume in the back of the hands. With volume loss, the tendons and veins become more prominent. Some people find their hands age faster than others due to factors such as genetics, sun exposure, health, frequent use of chemicals, smoking, and alcohol abuse.

Radiesse Restores Lost Volume

Now there is a fast and easy solution to turn back time for aging hands! Radiesse injections can restore the lost volume and at the same time stimulate new collagen production. Radiesse is a gel that has an active ingredient of calcium hydroxylapatite (CAHA), it is injected with lidocaine to reduce discomfort at the injection site.

Do Your Hands Give Away Your Age?

Dr. Miguel Delgado, M.D. states that many of his facelift patients feel their hands give away their age. Now that their face is refreshed and they are feeling younger, they are dismayed to see how their hands betray their face! With Radiesse, they can have immediate replenishment of volume loss with no allergy testing necessary. The results can be very long lasting; however, due to lifestyle, skin type, and muscle activity, results can vary.

Along with your Radiesse treatments, you can receive skin analysis from our Aesthetic RN, Ariel Kirk, who will address such issues as sun damage and those aging brown spots.

Call Now!

Call today (415) 898-4161 for a consultation with Dr. Delgado to find out if you are a good candidate for Radiesse.
